What to bring with you for your visit

Spine surgery is a specialty that depends heavily on imaging. In the "old days" most films, such as MRI and CT, were printed on large films sheets. Most imaging centers now can provide these MRIs, CTs and X-rays on CD. These images cannot be faxed or sent by email.

Please bring your most recent, relevant imaging studies to your visit on CD (preferably) or film sheets. Most films will have to have been done within the past year to be useful. Ask your imaging center for a copy on CD if you do not have one for your visit. Once these films are reviewed, they will be returned to you in most instances.

Please also bring radiology reports, EMG studies and reports of prior spine or neurosurgical procedures if you have them available. We may be able to obtain these reports by fax if you do not have copies of them.

We accept major credit cards, cash or checks for payments.

We accept most insurance plans. Please contact our office for any questions.

Wait times

When you come for your appointment, usually Dr. Sebastian is able to see you promptly. Sometimes, however, he runs behind schedule due to emergencies, patients arriving late or requiring more time. Dr. Sebastian has yet to discover how he can be in more than one place at the same time. We thank you for your understanding.
